This study was designed to estimate the incidence and assess the clinical significance of CN in cases of otitis media with effusion (OME) in children. It arises as a result of mechanical immobilization of the stirrup base in the oval window and disturbances in the self-resonance of the ossicular chain. The Carhart notch is not a true indication of "cochlear reserve" and this apparent bone conduction loss may be corrected by surgical intervention (Tonndorf, 1971). Carhart (1971) and Tonndorf (1971) provided a review of several theories to explain this phenomenon. For Carhart notch to be used as a preoperative predictor of stapes fixation, it should be shown that the notch exists with stapes fixation but not with other ossicular chain disorders, such as disconnection; however, few clinical investigations have assessed this issue.

Otosclerosis is a disease process of the ear that stiffens the stapes annular ligament and results in footplate immobilization. This produces a characteristic loss in bone-conducted (BC) hearing of about 20 dB between 1 and 2 kHz, known as "Carhart's notch", for which the specific mechanisms responsible have not yet been well understood. (1) The presence of a refraction of the bone conduction curve with a depth of 10 to 20 dB at a frequency of 2000 Hz (the so-called Carhart notch) in the presurgical tonal audiogram is an unfavorable prognostic factor in relation to closing the cochlear reserve and improving bone conduction after the stapedotomy.
